在PSP版《真·三国无双5》中，游戏会以系列作传统的战场动作为核心，而大幅强化“争霸模式”的内容。在本作中玩家除了以君主身分投身三国争霸之外，还能以武将身分来帮助主公完成一统天下的霸业。另外在游戏途中玩家可以随时改变自己的身分，像是从君主下野成为流浪武将，武将也可以下野转头其它君主挥下。体验刘备从起义群雄成为地方君主，又因败战而成为流浪武将，最后成为蜀汉皇帝的经历，或者是三姓家奴吕布不断背叛主君的武将生涯，因为PSP性能低，所以本作甘宁等武将原有“速游”特殊技能，现改为其它特殊技能。另外本作游戏动作画面也较模糊。
